What Constitutes an Ear Wax Emergency?

Not all ear wax requires urgent care. However, certain symptoms indicate a true emergency that needs same-day attention.

Sudden, complete hearing loss in one ear – This can be caused by a wax plug pressing against the eardrum. But it can also signal sudden sensorineural hearing loss (a medical emergency). We assess immediately and refer to A&E if needed.

Severe ear pain that came on suddenly – Impacted wax can cause pressure pain. However, pain with discharge or fever may indicate infection.

Water trapped behind a wax plug after swimming – This creates a "waterlogged" sensation and can rapidly lead to infection.

Flight-related ear barotrauma – Wax can prevent pressure equalisation during takeoff/landing, causing excruciating pain.

Our Emergency Protocol

  • Priority triage – Emergency calls answered immediately
  • Same-day slot – We reserve daily slots for urgent cases
  • Rapid consultation – 5-minute assessment, then immediate microsuction if safe
  • 24/7 hotline – Even after hours, we advise on next steps
  • Hospital referral – If we detect something serious (e.g., perforation, tumour), we arrange direct transfer to A&E
  • Medisave claimable for emergency procedures

Common Ear Emergencies in Singapore

✈️ Post-Flight Ear Block

Singaporeans travel frequently. After a long flight, many experience a "blocked ear" that won't clear. This is often wax that shifted during pressure changes.

The pain can be severe – worse than a typical earache. Microsuction provides instant relief by removing the wax and allowing pressure to equalise.

Do not try to force your ears to pop (Valsalva manoeuvre) if you suspect wax – you may rupture the eardrum.

🏊 Post-Swimming Water Trapping

Singapore's pools and beaches are popular. Sometimes water gets behind a wax plug, causing a sensation of liquid sloshing inside the ear.

This trapped water becomes a breeding ground for bacteria. Within 24–48 hours, it can develop into painful otitis externa (swimmer's ear).

Emergency microsuction removes both wax and water, preventing infection.

🩸 Cotton Bud Injury

Many Singaporeans still use cotton buds despite warnings. A sudden slip can push wax deep against the eardrum, causing intense pain and hearing loss.

Worse, the cotton tip can detach or the stick can perforate the eardrum. If you see blood or have severe pain after using a cotton bud, seek emergency care immediately.

We use microsuction to remove the impacted wax and examine for perforation.

🎧 Earbud Over-Insertion

With remote work and online learning, many use earbuds for hours daily. Pushing earbuds too deep can compact wax against the eardrum.

Symptoms include sudden muffled hearing, tinnitus, and a feeling of "something stuck". Emergency microsuction resolves this in minutes.

What to Do (and NOT Do) Before Your Emergency Visit

✅ Safe actions

  • Keep your ear dry – avoid swimming or showering without protection
  • Take over-the-counter pain reliever (paracetamol/ibuprofen) if needed – not aspirin if bleeding
  • Keep your head elevated to reduce pressure
  • Contact us immediately – early treatment prevents complications

❌ Dangerous actions (never do)

  • DO NOT use cotton buds – you will push wax deeper and risk perforation
  • DO NOT use ear candles – they cause burns and do nothing
  • DO NOT put water or oil drops – can worsen the blockage or introduce infection
  • DO NOT forcefully pop your ears – can rupture the eardrum if wax is impacted

🚨 Go directly to A&E (not our clinic) if you experience: Sudden complete hearing loss with dizziness, severe vertigo (spinning sensation), blood or pus draining from ear, facial weakness, or high fever. Call 995 for an ambulance.

What Happens During an Emergency Appointment

🔍 Step 1: Immediate Triage (5 min)

We ask about your symptoms, how they started, and any past ear problems. We check for red flags that require hospital referral.

📷 Step 2: Video Otoscope Exam (5 min)

You see exactly what we see – the wax plug, any swelling, discharge, or potential eardrum perforation. We explain the severity.

🔬 Step 3: Gentle Microsuction (10-15 min)

Using a microscope and low-pressure suction, we remove the wax. If the eardrum is perforated, we take extra care not to touch it. You will feel immediate relief as the wax is removed.

If there is severe infection, we may postpone full removal and prescribe antibiotics first.

📋 Step 4: Aftercare & Follow-up

We provide written instructions, ear plugs if needed, and schedule a follow-up (usually 5–7 days) to ensure healing. Emergency cases receive priority follow-up.

Emergency Ear Wax FAQs

How quickly can I be seen for an emergency?+

We reserve same-day emergency slots. Call our hotline before coming – we will triage over the phone and give you a time slot (usually within 2–4 hours). For true emergencies (sudden hearing loss with vertigo), we prioritise immediately.

What if my emergency happens after clinic hours?+

Our phone line is open 24/7 for advice. If it's after 10pm, we will guide you on whether to go to A&E or wait until morning. For severe symptoms (bleeding, sudden complete loss, facial weakness), go directly to the nearest hospital A&E.

Will emergency microsuction cost more?+

No. We charge the same as a standard microsuction ($80–$150). Medisave claims are processed the same way. No hidden "emergency fee".

Can you treat a perforated eardrum?+

If the eardrum is perforated (visible hole), we will NOT perform microsuction. We will refer you to an ENT surgeon at a hospital. However, if the perforation is small and there is wax blocking the canal, we may gently remove wax from the outer canal only – never near the perforation.

I have a foreign object (insect, bead) stuck in my ear – can you help?+

Yes, in many cases. We use microsuction or specialised instruments to remove foreign bodies. However, if the object is sharp or deeply embedded, we may refer to A&E for specialist removal under sedation. Call us first for advice.

What if the emergency is a child?+

We treat children for ear emergencies as well. Young children may have inserted small objects (beads, eraser tips) into their ears. We use paediatric instruments and gentle techniques. Parents stay beside the child throughout.
